Enrollment dates set for new students to Hazelwood School District

Parents of new students to the Hazelwood School District in the 2009-2010 school year can enroll during district-wide New Student Enrollment June 22 through July 10 at the HSD Learning Center at 15955 New Halls Ferry Road, Florissant.

Enrollment is by appointment only. Parents must bring a birth certificate and immunization records for each child, transcripts and/or grades, as well as proof of residency (deed of trust, previous year’s real estate tax receipt, a current lease and two billing statements) and a current driver’s license or state identification. If applicable, discipline records are needed for middle and high school students. Prospective kindergarten students must be age 5 before August 1 to enroll.

Guidance counselors will assist high school students with class schedules.

Representatives from the People’s Health Centers, Inc. will be available for immunizations and physicals on June 24 from 8:00 a.m. to 3:00 p.m.; July 1 from 9:00 a.m. to 12:00 p.m.; and July 8 from 8:00 a.m. to 3:00 p.m.

Hazelwood early childhood education offering registration dates for 2009-2010

The early childhood education program in the Hazelwood School District will begin registration for 2009-2010 in mid-June and continue through late July.

Parents of children age 3 and 4 years old should make an appointment to register on the following dates at an early childhood center location:

June 15 – June 19 Hazelwood East Early Childhood
June 22 – June 26 Hazelwood Central Early Childhood
July 13 – July 17 Hazelwood West Early Childhood
July 20 – July 24 Hazelwood Central Early Childhood

To schedule an appointment, please call (314) 953-4950. For convenience, families can register at any location during the summer dates. Beginning in August, families must make registration appointments at the early childhood center that will serve as their home school.

Parents and guardians should bring the following documents for registration: a birth certificate; deed, current lease, real estate tax receipt, mortgage statement or payment book, sales contract (must be replaced in 30 days with deed) or affidavit from the District Residency Office; two pieces of current mail (credit, utility or government related) addressed to enrolling parent(s) and delivered to resident on file by the U.S. Postal Service; immunization records that have been approved; copy of driver’s license or state identification card; and guardianship documents, if applicable.
